Referrals and Care Coordination

Thank you for being a community partner with Restore Therapy. We value our relationship with you. If you need to discuss a referral for a new patient you may get connected the following ways:

• Direct the patient to our New Patients page so they can begin the intake process.

• Send us your patient information with your release of information via secure email to: intakes@restoretherapypdx.com or fax: 503-765-1896

• If you need to speak to a clinician directly about the care of a current patient please call 503-376-7114 and dial their extension.

• Book a call with a Care Coordinator if you’d like to speak with us on the phone.
